LTF Case Manager

Title: Latino Task Force (LTF) Case Manager

  • Location: San Francisco, CA. This position will rotate across all LTF Resource Hubs: Casa De Apoyo in Excélsior, The Hut at Executive Park in Visitacion Valley and our flagship Hub in the Mission District

  • Reports to: Manager of Case Management, and Public Benefits, Mario Flores

  • Job Status: Full-time (M-F, 40hrs/week)

  • Compensation: $68,640 / salary; exempt

The Case Manager role provides comprehensive wrap-around support for all LTF clients. This includes but is not limited to housing, income, public benefit, health, immigration/legal, translation, and food assistance. The Case Manager handles crisis situations if necessary, and directs clients to appropriate services. Additionally, Case Managers are responsible for writing Case Manager Letters of support. Therefore, strong written as well as verbal communication in both English and Spanish is required.

Job Duties:

  • Serve 7-9 clients daily at our recovery hubs conducting various assessments of clients; assess their basic needs, identify barriers, guide them to appropriate applications/programs

  • Conduct triage case management that includes identifying priority areas of focused and immediate next steps to address critical needs

  • Responsible for maintaining case notes on each client; accurately and regularly  input data into spreadsheets and funder databases to maintain participant  information in accordance with agency standards and any program-specific  funder(s)’ requirements  

  • Make referrals to other services and programs as appropriate; attend and/or  bring clients to such activities when necessary.  

  • Engage with diverse clientele using appropriate translation tools when necessary 

  • When needed, attend school meetings, court hearings and other appointments  influencing the clients; act as advocate when appropriate  

  • Responsible for writing character and recommendation letters on behalf of clients 

  • Work closely with LTF partners at large like academic and career coaches,  housing team, public benefits, and others to provide wrap-around support for  clients 

  • Ensure data/documentation collection and reporting is complete for each client  on your caseload

Qualifications:

  •  Fully bilingual (English/Spanish), both verbal and written

  • At least 1-2 years experience in the Case Management, Public Benefits or related field field

  • Ability to work effectively with diverse populations, particularly with immigrants and non-English speakers

  • Serve 7-9 clients daily at our recovery hubs conducting various assessments of clients; assess their basic needs, identify barriers, guide them to appropriate applications/programs

  • Conduct triage case management that includes identifying priority areas of focused and immediate next steps to address critical needs

  • Must have a valid CA Driver’s License and Vehicle

  • Demonstrated ability to maintain confidentiality and uphold a high level of integrity

  • Flexible and adaptable to variable work schedules and rotating locations

  • Skilled in setting goals and maintaining focus amid changing challenges

  • Experience in advocating for clients rights and access to resources in education, housing, healthcare, and public benefits.

Education and Experience:

Required:

  • High school GED/Diploma

  • At least 2 years experience working as a case manager or public benefit specialist or related fields

  • 1-2  years experience working or volunteering with Non-profits serving families or youth in the SF Bay Area.

Preferred:

  • Bachelor's degree 

  • Ability to assess crisis situations and initiate action independently and without direction

  • Maintains strong connections within the social services sector, with a well-established network of community partners and resource providers

LTF Career Coach

Title: Latino Task Force (LTF) Career Coach

  • Location: San Francisco, CA This position will rotate across all LTF Resource Hubs: Casa De Apoyo in Excélsior, The Hut at Executive Park in Visitacion Valley and our flagship Hub in the Mission

  • Reports to: WORKFORCE MANAGER, Jennifer Robles

  • Job Status: Full-time (M-F, 40hrs/week)

  • Compensation: Full-time, $28/hour

The LTF Career Coach ensures participants have an understanding of high-growth sectors, careers, training and education options while supporting immediate employment opportunities. Career Coaches work with participants one on one in order to create participant-centered career and education plans, which are designed to help them reach the short and long-term goals that will lead to economic self-sufficiency. Target populations that will be served through this role are low income, veterans, homeless, immigrant, re-entry, dislocated workers and individuals who are seeking employment services who present significant barriers to employment.

Job Duties:

  • Meet with participants at assigned LTF resource hub

  • Meet with participants in person on a regular basis to formally monitor the progress of gaining employment; check in via phone call, text, and email on a daily to weekly basis for informal monitoring

  • Create and establish client tracking system to maintain effective supportive services

  • Connect with local employers to foster long-term job placements for participants; make referrals and/or connections for participants; inform, attend and/or bring participants to such activities when necessary

  • Development of resumes, cover letters and applications; advises job seekers on employability skills; works with job seekers to gain relevant skills; helps job seekers explore alternative employment options

  • Conduct industry-focused assessments with participants to gauge their readiness in various career pathways

  • Troubleshoot work site issues with participants, support and empower them to resolve workplace conflicts, and provide one on one training as needed

  • Meet frequently with other LTF content managers and staff to ensure collaborative case planning and assessment

  • Attend weekly program meetings, monthly agency staff meetings, and individual weekly supervision with the workforce manager as scheduled

  • Work with external service providers to ensure participants develop and maintain their educational, career and wellness plans

  • Position not available for remote work

Qualifications:

  • Fully bilingual (English/Spanish), both verbal and written

  • Ability to work full time (Mon-Fri, possible to work some evenings and periodic weekends)

  • Ability to be on call and support with team coverage as needed

  • Proficient computer skills, required including competency with Microsoft Office and Google Suite. Including experience with data entry and analysis

  • At least 1-2 years experience in the workforce field

  • At least 1-2 years experience working with multi-barrier adults ages 25-95 years old

  • Successful completion of Live Scan/fingerprinting (upon hire)

Preferred:

  • Bachelor’s degree or attending college currently

  • Knowledgeable of Bay Area communities, careers and education resources

  • Ability to assess crisis situations and initiate action independently and without direction

  • Familiarity working with undocumented transitional aged youth and adults in the Bay Area

  • Maintains strong connections within the workforce sector, with a well-established network of community partners and resource providers
