Located across our five Latino Task Force Resource Hubs, the LTF Housing Team collaborates with multiple entities that aim to provide resources to community members facing housing challenges. These resources can be as follows:

Assist people to apply for rental relief through the local and state programs (Housing Is Key).

Assist people to apply to DAHLIA San Francisco Housing Portal.

Refer individuals to legal aid when necessary to learn more about housing protections, etc.

Collaborate with other community organizations to conduct training, workshops, and webinars to benefit the community.

LTF Transitioning Housing Program (Internal Process)
The San Francisco Latino Task Force has allocated resources and efficient tools to serve families and individuals facing housing instability. Through the LTF Resource Hubs system, the Housing Unit will implement this program through a referral system that will be linked to Case Management to make sure that eligible people can navigate the housing market as smoothly as possible. In addition, this project gives the Housing Team an exciting opportunity to support community members to improve their living situation.
The LTF Transitioning Housing Program is referral-based. Referrals will come directly from the resource hubs. Each resource hub will have staff members with access to the referral form. Once a referral has been made, they will be placed with a case manager to assess their housing needs and evaluate how our team can be of service and if the applicant is eligible.
What is the Transitioning Housing Program (THP)?
The Transitioning Housing Program will serve as a pipeline system to bridge individuals or families to lease available units on the private market across the Bay Area. This program will consist of case management, housing negotiation, financial literacy, etc.
This program is referral-based, meaning you cannot apply for the Transitioning Housing Program through a separate paper application or a link application (only through the LTF process listed above). Instead, LTF staff members will refer eligible individuals/families to the Transitioning Housing program.
Individuals who become part of the THP will receive support searching for housing in the private rental market in the San Francisco/Bay Area. Individuals can potentially receive financial assistance for move-in costs, such as security deposits, etc. Any rental unit negotiated through the THP will be clean, safe for habitation, and ready for move-in. Additionally, individuals will be able to receive voluntary ongoing supportive services related to housing or connections to other resources, such as help communicating with a landlord or job search assistance (services provided through the Latino Task Force Hubs).
I need emergency housing tonight. Is this program for me?
This program is called “Transitioning Housing” because it aims to serve the community as a bridge between tenants and landlords through negotiation and case management. This program will have the tools to transition individuals to available units. Please note that it can take several months between the application process and move-in. This program is not a first-come-first-served program but a priority program based on a point system. If you are currently experiencing homelessness and are interested in accessing other resources, there might be other available options through other organizations.
Who can be referred to the THP?
The LTF internal Transitioning Housing Program is limited to families and individuals who are:

Current LTF Resource Hubs clients who have received services within the last three months.

Households that must have a source of steady income. Please note that this is not a subsidized program.

Currently experiencing unstable housing, i.e., homelessness, couch-surfing, at risk of experiencing homelessness.
Internal Priorities
This program is referral-based, meaning individuals cannot apply directly using a paper application or a link application; instead, LTF staff members identify and refer specific individuals who have been receiving services in the hubs in the past three months. The Housing Unit has developed this process to align with our agency’s strategic framework and equity goals to serve individuals across the LTF Resource Hubs system.
How can I get referred?
The LTF Transitioning Housing Program is referral-based. Referrals will come directly from the resource hubs. Each resource hub will have LTF staff members access to the referral form. After the LTF staff submits the referral form, a staff member from the Housing Unit will reach out to a limited number of people to schedule appointments with Case Management to assess their housing needs and evaluate how our team can be of service. Therefore, the referrals are not first-come, first-serve.
